BEFORE YOU BEGIN: Please make sure you have the following information on hand before you begin the application process. If you have all the necessary information, completing the application should take about 5-10 minutes.

Required Business Information for Application

General business & deposit information:

• Business Information - addresses, telephone number, fax number, email address, website (if applicable)
• Business License / Articles of Incorporation (if incorporated)
• Current credit card processor information (if applicable)
• VOID Cheque for deposits
• 3 months previous processing history (if available)

Information about your business:

• Totals of annual cash and credit card sales, and percentage of credit card sales by mail order, telephone order and in store purchases
• Store refund/exchange policy with respect to credit cards
• Delivery information if mail order/telephone order

Ownership information

• Names, addresses, phone numbers of first/second owner, partner, or officer
